Job summary

Homeside Rehab is seeking compassionate and motivated Physical Therapist (PT) or Physical Therapy Assistant (PTA) to provide one-on-one home-based therapy services in Sundown, NY 12740.

Qualifications

• Active New York State Physical Therapist (PT) or Physical Therapy Assistant (PTA) license/certification.
• Basic computer proficiency for electronic documentation.
• Strong interpersonal and communication skills.
• Ability to work independently in a home-based setting.
• Valid driver''s license and reliable transportation for home visits.
• Home health, outpatient, rehabilitation, or skilled nursing experience preferred.
• Minimum of 1 year of clinical experience preferred.

Responsibilities

• Provide skilled Physical Therapy services in the patient's home.
• Evaluate patients and develop individualized plans of care, as applicable to your professional scope of practice.
• Carry out established treatment plans and therapeutic interventions.
• Provide functional activities, therapeutic exercises, ADL training, strengthening, coordination activities, and other interventions within your professional scope.
• Monitor patient progress and communicate changes or concerns to the appropriate supervising therapist and clinical team.
• Maintain accurate and timely clinical documentation in accordance with company and regulatory requirements.
• Communicate professionally with patients, caregivers, therapists, and office staff.
• Provide one-on-one therapy services in the patient's home.

Job Details

Pay: $45–$70 per hour, based on discipline, experience, and case assignment
Schedule: Flexible – choose your availability
Shifts: Day and evening availability, Monday through Friday, with optional weekend availability
