Job summary

Homeside Rehab is seeking compassionate Occupational Therapists (OTs) and Occupational Therapy Assistants (OTAs) to provide one-on-one home-based therapy services in Sundown, NY 12740. Therapists enjoy flexible scheduling, competitive compensation, and the opportunity to improve patients' independence and quality of life.

Responsibilities include evaluating patients, developing plans of care, carrying out treatment plans, and maintaining documentation.

Qualifications

• Active New York State OT or OTA license/certification
• Basic computer proficiency for electronic documentation
• Strong interpersonal and communication skills
• Ability to work independently in a home-based setting
• Valid driver’s license and reliable transportation for home visits
• Home health/outpatient rehabilitation or skilled nursing experience preferred
• Minimum of 1 year of clinical experience preferred

• Provide skilled Occupational Therapy services in the patient's home

• Carry out established treatment plans and therapeutic interventions
• Provide functional activities, therapeutic exercises, ADL training, strengthening, coordination activities, and other interventions within your scope
• Monitor patient progress and communicate changes or concerns to supervising therapist and clinical team
• Maintain accurate and timely clinical documentation in accordance with company and regulatory requirements
• Communicate professionally with patients, caregivers, therapists, and office staff
• Provide one-on-one therapy services in the patient's home

Job Responsibilities

• Provide skilled Occupational Therapy services in the patient's home

• Evaluate patients and develop individualized plans of care, as applicable to your professional scope of practice

• Carry out established treatment plans and therapeutic interventions

• Provide functional activities, therapeutic exercises, ADL training, strengthening, coordination activities, and other interventions within your professional scope

• Monitor patient progress and communicate changes or concerns to the appropriate supervising therapist and clinical team

• Maintain accurate and timely clinical documentation in accordance with company and regulatory requirements

• Communicate professionally with patients, caregivers, therapists, and office staff

• Provide one-on-one therapy services in the patient's home

Job Details

Pay: $45–$70 per hour, based on discipline, experience, and case assignment
Schedule: Flexible – choose your availability
Shifts: Day and evening availability, Monday through Friday, with optional weekend availability
