Job summary

Lorian Health in Riverside is seeking a qualified Physical Therapist to provide home-based PT services prescribed by the attending physician. The PT will direct, supervise, evaluate, and deliver care to patients in their homes.

Requirements include graduation from an approved physical therapy program, licensure in the state, CPR certification, and typically 2+ years of PT experience. A valid driver's license and reliable vehicle are required.

Qualifications

• Graduate of a physical therapy program approved by applicable medical education councils
• Licensed or registered to practice as a PT in the state of practice
• Two or more years of PT experience preferred
• Possesses CPR certification and maintains health clearance
• Ability to read, follow written instructions, and document care
• Strong self-direction and ability to work with minimal supervision
• Valid driver’s license and insured vehicle required
• Ability to communicate verbally in client language and document activities

Responsibilities

• Provide treatment as ordered by the attending physician, dentist, or podiatrist
• Develop and update the plan of treatment (POC)
• Evaluate level of function in collaboration with the physician
• Record and report patient conditions to the physician, case manager, and health records
• Instruct and consult family and other staff on therapy programs
• Supervise other healthcare personnel when needed
• Assess home environment and make recommendations
• Participate in in-service education programs
• Complete and submit accurate clinical notes per policy

Job Summary

The Physical Therapist (PT) is a qualified professional who directs, supervises, evaluates, and provides physical therapy services to patients/clients in the home as prescribed by the attending physician.

Qualifications

• Graduate of a physical therapy school approved by the Council of Medical Education of the AMA in Association or its equivalent.
• Licensed or registered to practice as a PT in the state of practice.
• Preferably at least two (2) years of experience as a PT.
• Possesses and maintains good physical stamina and mental health. Presents a pre-employment physician’s health clearance, including negative TB skin tests and/or CXR and other tests as required by specific state statute.
• Has the ability to read and follow written instructions and document care given.
• Is self-directing and has the ability to work with little direct supervision.
• Possesses and maintains current CPR certification.
• Is flexible and cooperative in fulfilling the role obligations.
• Demonstrates tact, patience, and good personal hygiene.
• Is a licensed driver and has an automobile that is insured in accordance with Lorian Health (LORIAN) requirements.
• Must be able to verbally communicate in the language of the client and document all client-related activity practices.

Responsibilities

• Providing treatment as ordered by the attending physician, dentist, or podiatrist.
• Assisting in developing and updating the plan of treatment or Plan of Care (POC).
• Assisting the physician in evaluating level of function.
• Observing, recording, and reporting information on the patient’s condition to the attending physician, case manager, and in the patient’s health record.
• Advising, consulting, and when appropriate, instructing family and other LORIAN personnel in patient’s therapy program.
• Teaching and supervising other healthcare personnel when appropriate.
• Evaluating the home environment and making appropriate recommendations.
• Participating in in-service education programs.
• Completes, maintains, and submits accurate and relevant clinical notes regarding patients/clients condition and care given in accordance with LORIAN policies and procedures.

This position is based on a Pay per visit compensation model.

Pay per visit ranges from $100 to $150 visit commensurate with experience and depending on type of visit.
