Job summary

Lorian Health in Long Beach, CA, seeks a licensed physical therapist for home health services. You will implement treatments, update care plans, and evaluate function in patients' homes, coordinating with physicians and case managers.

Requirements include 1 year in home health, AMA-approved PT education, and active state licensure, plus CPR and a valid driver’s license. Pay-per-visit and a startup bonus are offered along with strong benefits.

Qualifications

• Minimum 1 year of recent Home Health or Hospice experience.
• Graduate of a physical therapy school approved by the AMA or equivalent.
• Licensed or registered to practice as a PT in the state of practice.
• Ability to read and follow written instructions and document care given.
• Self-directed with ability to work with little direct supervision.
• Current CPR certification.
• Licensed driver with insured automobile per Lorian Health requirements.
• Must be able to verbally communicate in the language of the client and document activities.

Responsibilities

• Providing treatment as ordered by the attending physician, dentist, or podiatrist.
• Assisting in developing and updating the Plan of Treatment or POC.
• Assisting the physician in evaluating level of function.
• Observing, recording, and reporting information on the patient’s condition to the attending physician and case manager.
• Advising, consulting, and instructing family and other LORIAN personnel in therapy program.
• Teaching and supervising other healthcare personnel when appropriate.
• Evaluating the home environment and making appropriate recommendations.
• Participating in in-service education programs.

Schedule:

Monday- Friday with weekend rotations

Qualifications

• Minimum of one (1) year of recent (within the last three (3) years) experience in a Home Health or Hospice Setting.
• Graduate of a physical therapy school approved by the Council of Medical Education of the AMA in Association or its equivalent.
• Licensed or registered to practice as a PT in the state of practice.
• Has the ability to read and follow written instructions and document care given.
• Is self-directing and has the ability to work with little direct supervision.
• Possesses and maintains current CPR certification.
• Is a licensed driver and has an automobile that is insured in accordance with Lorian Health (LORIAN) requirements.
• Must be able to verbally communicate in the language of the client and document all client-related activity practices.

This position is based on a pay per visit compensation model. Pay per visit ranges from $100 to $150 / visit commensurate with experience and depending on type of visit.
