Job summary

Lorian Health is seeking a licensed Physical Therapist to provide home-based PT services in the San Jose area. You will direct, supervise, evaluate, and administer therapy per physician orders in patients' homes.

Qualifications include AMA-approved PT education, valid PT license in the state, CPR cert, and 2+ years of experience preferred. A driver and insured vehicle are required; strong communication is essential.

Qualifications

• Graduate of a physical therapy program approved by the AMA or equivalent.
• Licensed to practice as a PT in the practicing state.
• Preferably 2+ years of PT experience.
• CPR certification maintained.
• Must have a valid driver's license and insured vehicle.
• Able to read and follow written instructions and document care.

Responsibilities

• Provide treatment as ordered by attending physician, dentist, or podiatrist.
• Assist in developing and updating the plan of care (POC).
• Evaluate level of function with physician oversight.
• Record and report patient status to physician and case manager.
• Instruct family and other staff in the therapy program.
• Supervise other healthcare personnel when appropriate.
• Evaluate home environment and make recommendations.
• Complete accurate clinical notes per policies.

Job Summary

The Physical Therapist (PT) is a qualified professional who directs, supervises, evaluates, and provides physical therapy services to patients/clients in the home as prescribed by the attending physician.

Qualifications

• Graduate of a physical therapy school approved by the Council of Medical Education of the AMA in Association or its equivalent.
• Licensed or registered to practice as a PT in the state of practice.
• Preferably at least two (2) years of experience as a PT.
• Possesses and maintains good physical stamina and mental health. Presents a pre-employment physician’s health clearance, including negative TB skin tests and/or CXR and other tests as required by specific state statute.
• Has the ability to read and follow written instructions and document care given.
• Is self-directing and has the ability to work with little direct supervision.
• Possesses and maintains current CPR certification.
• Is flexible and cooperative in fulfilling the role obligations.
• Demonstrates tact, patience, and good personal hygiene.
• Is a licensed driver and has an automobile that is insured in accordance with Lorian Health (LORIAN) requirements.
• Must be able to verbally communicate in the language of the client and document all client-related activity practices.

Responsibilities

• Providing treatment as ordered by the attending physician, dentist, or podiatrist.
• Assisting in developing and updating the plan of treatment or Plan of Care (POC).
• Assisting the physician in evaluating level of function.
• Observing, recording, and reporting information on the patient’s condition to the attending physician, case manager, and in the patient’s health record.
• Advising, consulting, and when appropriate, instructing family and other LORIAN personnel in patient’s therapy program.
• Teaching and supervising other healthcare personnel when appropriate.
• Evaluating the home environment and making appropriate recommendations.
• Participating in in-service education programs.
• Completes, maintains, and submits accurate and relevant clinical notes regarding patients/clients condition and care given in accordance with LORIAN policies and procedures.

This position is based on a Pay per visit compensation model.

Pay per visit ranges from $100 to $150 visit commensurate with experience and depending on type of visit.
