Job summary

Lorian Health is seeking a licensed Physical Therapist to provide patient-centered home health services in Stockton and surrounding areas. You will implement and update Plan of Care, evaluate functional status, and document patient progress in the health record.

Role requires 1+ year in home health or hospice, AMA-approved PT degree, active license, and CPR. You will collaborate with physicians, case managers, and family to optimize home-based outcomes and safety.

Qualifications

• Minimum of one (1) year of recent (within the last three (3) years) experience in a Home Health or Hospice Setting.
• Graduate of a physical therapy school approved by the AMA Council of Medical Education (or equivalent).
• Licensed or registered to practice as a PT in the state of practice.
• Ability to read and follow written instructions and document care.
• Self-directing with ability to work with little supervision.
• CPR certification.
• Driver’s license with insured vehicle.
• Must be able to verbally communicate in the language of the client.

Responsibilities

• Providing treatment as ordered by the attending physician, dentist, or podiatrist.
• Assisting in developing and updating the plan of treatment or Plan of Care (POC).
• Assisting the physician in evaluating level of function.
• Observing, recording, and reporting patient condition information to the attending physician, case manager, and health record.
• Advising, consulting, and instructing family and other LORIAN personnel in the patient’s therapy program.
• Teaching and supervising other healthcare personnel when appropriate.
• Evaluating the home environment and making appropriate recommendations.
• Participating in in-service education programs.

• Is self-directing and has the ability to work with little direct supervision.
• Possesses and maintains current CPR certification.
• Is a licensed driver and has an automobile that is insured in accordance with Lorian Health (LORIAN) requirements.
• Must be able to verbally communicate in the language of the client and document all client-related activity practices.

This position is based on a pay per visit compensation model. Pay per visit ranges from $110 to $170 / visit commensurate with experience and depending on type of visit.
