As a PT in our PACE program, you will build long-term therapeutic relationships with elderly participants, helping them maintain independence and remain in their communities. You will provide comprehensive physical therapy evaluations and treatments for PACE participants in a community-based setting. You will collaborate with an interdisciplinary team including physicians, nurses, social workers, and occupational therapists to develop holistic care plans.
