Job summary

Blue Ridge Care invites a dedicated Physical Therapist to join our PACE program, delivering patient-centered care across the PACE Center, homes, and community settings.

You will evaluate mobility, craft individualized plans, and collaborate with physicians, nurses, social workers, and therapists to maximize independence and safety for older adults. Mentoring staff and contributing to quality initiatives are part of the role.

Qualifications

• Must hold an active Physical Therapist license.
• CPR certification required.
• Strong communication and teamwork skills are essential.
• Driver's license and reliable transportation are required.

Responsibilities

• Evaluate participants' strength, mobility, balance, and functional abilities.
• Develop individualized treatment plans to promote independence and safety.
• Provide therapy services at the PACE Center, in participants' homes, and in the community.
• Collaborate with the Interdisciplinary Team to create holistic care plans.
• Educate participants and caregivers on mobility, fall prevention, and the use of adaptive equipment.
• Supervise and mentor Physical Therapy Assistants and rehab staff.

Every day, you'll help older adults maintain their independence, improve their mobility, prevent functional decline, and continue living safely in the communities they call home. Your expertise will directly impact participants' quality of life while supporting a care model that keeps seniors where they want to be: active, engaged, and independent.

As part of an interdisciplinary team, you'll collaborate with physicians, nurses, social workers, occupational therapists, and other healthcare professionals to provide comprehensive, participant-centered care.

What You'll Do

You'll make a meaningful difference by:

• Evaluating participants' strength, mobility, balance, and functional abilities

• Developing individualized treatment plans that promote independence and safety

• Providing therapy services in the PACE Center, participant homes, and community settings

• Collaborating with the Interdisciplinary Team to create holistic care plans

• Educating participants and caregivers on mobility, exercise, fall prevention, and adaptive equipment

• Recommending and evaluating durable medical equipment, orthotics, prosthetics, and assistive devices

• Supervising and mentoring Physical Therapy Assistants and rehabilitation staff

• Supporting quality improvement initiatives that enhance participant outcomes

• Helping seniors achieve their highest level of function and quality of life

Required Qualifications

• Graduate of an accredited Physical Therapy program

• Current Physical Therapist license

• Current CPR certification

• Valid driver's license and reliable transportation

Preferred Qualifications

• Experience working with frail older adults or medically complex populations

• Experience in an interdisciplinary healthcare environment

• Strong clinical assessment, treatment planning, and communication skills

• Knowledge of rehabilitation practices, adaptive equipment, mobility training, and fall prevention
