Centra Healthcare Solutions Inc. is seeking an experienced or newly graduated Physical Therapist to join our team on a contract assignment in the United States.

The role involves delivering skilled PT services in SNF/LTC settings, following physician orders and developing patient care plans. Requirements include graduation from an accredited PT program, current state PT licensure or licensure in process, and active CPR certification.

Qualifications

• Must have graduated from an accredited Physical Therapy program.
• Current state license as a Registered Physical Therapist (PT) or actively pursuing licensure.
• Current CPR certification.
• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services per physician orders.
• Assess, plan, evaluate and modify care to meet goals of physical therapy interventions.
• Collaborates with all disciplines to plan and evaluate team goals for each patient.

A great SNF / LTC is looking for an experienced or newly graduated Physical Therapist / PT to join their growing team for a contractassignment! This is a contract position. Days schedule. This role assumes responsibility and accountability for incorporating the vision, values, mission and critical goals of the organization into their job performance.
