Job summary

Centra Healthcare Solutions Inc is seeking an experienced or newly graduated Physical Therapist for a contract assignment in Helena, MT. The PT will join a great SNF/LTC team, assume responsibility for delivering high-quality care, and align with the organization's mission and goals.

The role requires graduation from an accredited PT program, active or in-process licensure, CPR certification, and the ability to develop patient care plans and collaborate with multidisciplinary teams.

Qualifications

• Must have graduated from an accredited physical therapy program.
• Current state license or in process for licensure.
• Current CPR certification.
• Develop a plan of care for each patient.
• Provide skilled physical therapy services per physician orders.
• Assess and modify care to meet PT goals.
• Collaborate with all disciplines to plan and evaluate team goals.

A great SNF / LTC is looking for an experienced or newly graduated Physical Therapist / PT to join their growing team for a contractassignment! This is a contract position. Days schedule. This role assumes responsibility and accountability for incorporating the vision, values, mission and critical goals of the organization into their job performance.

Requirements include:

• Must have graduated from an accredited school.
• Current state license as a Registered Physical Therapist / PT or proactively in the process of the application process for current state licensure as a Registered Physical Therapist / PT.
• Current CPR certification.
• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services / interventions in accordance with physician orders.
• Assesses patient needs, plans for, evaluates and modifies care to meet goals of physical therapy interventions.
• Collaborates with all disciplines to plan and evaluate team goals for each patient.
