Job summary

Centra Healthcare Solutions Inc in Ashland, Wisconsin is seeking an experienced or newly graduated Physical Therapist for a contract assignment. The role is a full-time contract position with a daily schedule and accountability for integrating the organization's mission into daily work.

The successful candidate will develop care plans for each patient, provide skilled PT services per physician orders, and collaborate with multidisciplinary teams to achieve patient goals while maintaining CPR

Qualifications

• Must have graduated from an accredited PT program and hold active PT licensure.
• Current CPR certification required.
• Develop a plan of care for each physical therapy patient and provide skilled PT services per physician orders.
• Collaborate with multidisciplinary team to achieve patient goals.

Responsibilities

A great SNF / LTC is looking for an experienced or newly graduated Physical Therapist / PT to join their growing team for a contract assignment!This is a contract position.Days schedule.This role assumes responsibility and accountability for incorporating the vision, values, mission and critical goals of the organization into their job performance.

Requirements include: Must have graduated from an accredited school.Current state license as a Registered Physical Therapist / PT or proactively in the process of the application process for current state licensure as a Registered Physical Therapist / PT.Current CPR certification.Develop a plan of care for each physical therapy patient.Provide skilled physical therapy services / interventions in accordance with physician orders.Assesses patient needs, plans for, evaluates and modifies care to meet goals of physical therapy interventions.Collaborates with all disciplines to plan and evaluate team goals for each patient.
