Job summary

Centra Healthcare Solutions Inc is seeking an experienced or newly graduated Physical Therapist to join a SNF/LTC facility on a contract assignment, with a day schedule.

This role requires developing plans of care, delivering skilled PT interventions per physician orders, and assessing and adjusting care to meet patient goals. A state license and CPR certification are required to practice.

Qualifications

• Must have graduated from an accredited school.
• Current state license as a Registered Physical Therapist / PT or in process for licensure.
• Current CPR certification.

Responsibilities

• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services / interventions in accordance with physician orders.
• Assesses patient needs, plans for, evaluates and modifies care to meet goals of physical therapy interventions.
• Collaborates with all disciplines to plan and evaluate team goals for each patient.
