Job summary

Centra Healthcare is seeking a licensed Physical Therapist to join a SNF/LTC setting in Fresno, CA for a contract assignment. This role requires licensure in the state, CPR, and ongoing licensure status, with a contract schedule of days.

The PT will implement and adjust plans of care, provide skilled PT services per physician orders, and assess patient needs while collaborating with all disciplines to achieve team goals and organizational objectives.

Qualifications

• Must have graduated from an accredited school.
• Current state license as a Registered Physical Therapist / PT or in process for licensure.
• Current CPR certification.
• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services / interventions per physician orders.
• Assesses patient needs, plans for, evaluates and modifies care to meet goals of physical therapy interventions.
• Collaborates with all disciplines to plan and evaluate team goals for each patient.

A great SNF / LTC is looking for an experienced or newly graduated Physical Therapist / PT to join their growing team for a contract assignment!

• This is a contract position.
• Days schedule.
• This role assumes responsibility and accountability for incorporating the vision, values, mission and critical goals of the organization into their job performance.

Requirements include:

• Must have graduated from an accredited school.
• Current state license as a Registered Physical Therapist / PT or proactively in the process of the application process for current state licensure as a Registered Physical Therapist / PT.
• Current CPR certification.
• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services / interventions in accordance with physician orders.
• Assesses patient needs, plans for, evaluates and modifies care to meet goals of physical therapy interventions.
