Centra Healthcare is seeking a licensed Physical Therapist (PT) in Stockton, CA for a contract assignment. The role requires licensure, CPR certification, and the ability to develop and implement care plans while providing skilled PT services per physician orders.

Qualifications

• Graduated from an accredited school.
• Current state license as a Registered Physical Therapist or in licensure process.
• Current CPR certification.
• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services / interventions in accordance with physician orders.
• Assess patient needs, plan, evaluate and modify care to meet goals of physical therapy interventions.
• Collaborates with all disciplines to plan and evaluate team goals for each patient.

A great SNF / LTC Facility is looking for an experienced or newly graduated Physical Therapist / PT to join their growing team for a contract assignment!

• This is a contract position.
• Days schedule.
• This role assumes responsibility and accountability for incorporating the vision, values, mission and critical goals of the organization into their job performance.

Requirements include:

• Must have graduated from an accredited school.
• Current state license as a Registered Physical Therapist / PT or proactively in the process of the application process for current state licensure as a Registered Physical Therapist / PT.
• Current CPR certification.
