Centra Healthcare is seeking a Physical Therapist (PT) for a contract assignment in Lancaster, CA. Must be licensed in California and CPR certified; you will develop a plan of care and provide skilled PT services per physician orders.

This contract role offers a days schedule with a weekly pay range of 1,931.20 – 2,272.00 and comprehensive benefits through Centra Healthcare.

Qualifications

• Must have graduated from an accredited school
• Current state license as a Registered Physical Therapist / PT or in process for licensure
• Current CPR certification

Job Description:

A great SNF / LTC Facility is looking for an experienced or newly graduated Physical Therapist / PT to join their growing team for a contract assignment!

• This is a contract position.


• Days schedule.


• This role assumes responsibility and accountability for incorporating the vision, values, mission and critical goals of the organization into their job performance.

Requirements include:

• Must have graduated from an accredited school.


• Current state license as a Registered Physical Therapist / PT or proactively in the process of the application process for current state licensure as a Registered Physical Therapist / PT.


• Current CPR certification.


• Develop a plan of care for each physical therapy patient.


• Provide skilled physical therapy services / interventions in accordance with physician orders.


• Assesses patient needs, plans for, evaluates and modifies care to meet goals of physical therapy interventions.


• Collaborates with all disciplines to plan and evaluate team goals for each patient.
