Job Position:

Physical Therapist for an In-house Skilled Nursing Facility

Job Type:

Full Time

Job Purpose:

To assist in planning, organizing, developing, and directing Physical Therapy services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by the Administrator, to ensure the highest quality resident care.

Job Functions:

1. Provide physical therapy services to residents according to established standards, procedures, and productivity standards.
2. Adhere to documentation and billing procedures for physical therapy services.
3. Evaluate, plan, implement, and follow-up on resident therapy programs with sound judgment.
4. Conduct regular screenings to determine residents' need for intervention/treatment.
5. Evaluate residents for treatment planning and implementation.
6. Develop treatment plans with clear short and long-term goals.
7. Communicate evaluation findings and treatment recommendations to residents, families, and team members.
8. Complete all required documentation in compliance with CMS standards using NetHealth EMR systems.
9. Perform other duties as assigned to support organizational goals.

Education & Requirements:

1. Bachelor of Science Degree in Physical Therapy from an accredited program.
2. Active Physical Therapy license in the state(s) of practice.
3. Ongoing continuing education to maintain licensure and certification.
