Pleasant Acres Rehabilitation and Nursing Center is looking for compassionate and skilled PRN Physical Therapist to join our in-house rehab team!

Responsibilities:
  • Plan treatment for residents and patients with temporary or long-term disabilities to relieve pain, restore or improve function, and promote healing.
  • Assure all treatment is delivered in accordance with an established plan of care.
  • Provide clinical support and instruct patients, families, and caregivers.
  • Monitor patient response to treatment intervention.
  • Complete required forms and documentation in accordance with company policy and state/federal regulations.
  • Consult with patients to learn about their physical condition.
  • Assess and interpret patient evaluations and test results.
  • Develop treatment plans using a variety of treatment techniques.
  • Create personalized fitness-oriented health care programs for patients.
  • Administer medically prescribed physical therapy treatments to relieve pain and improve mobility.
  • Advise patients on exercise techniques.
  • Advise patients and their families about in-home treatment options.
  • Provide educational information about injury prevention, ergonomics, and ways to promote physical health.
  • Consult and collaborate with other healthcare professionals.
  • Document patient care history.
  • Comply with rules, regulations, and procedures.
