*$10,000 Sign On Bonus for Full Time*

BAYADA Home Health Care is currently seeking an experienced Physical Therapist, PT, for a Full Time or Part Time opportunity performing home health visits throughout Erie, PA. This office services adult and geriatric clients on a per visit basis.

One year of prior clinical experience as a licensed Physical Therapist is required. As a home care Physical Therapist, you will be an integral member of a multidisciplinary healthcare team that provides rehabilitative care and skilled nursing to clients, enabling them to receive necessary medical care to remain at home.

Responsibilities include:

1. Make home visits to clients in designated geographic areas.
2. Perform diagnostic tests and measurements, including mobility, joint range of motion, transfer status, stability, ambulation patterns, muscle strength and endurance, balance, and safety assessments.
3. Implement protective interventions to prevent wound formation and promote wound healing using evidence-based dressings and topical agents.
4. Develop and implement individualized care plans, including manual therapeutic exercises, gait training, and balance interventions.
5. Assess and revise care plans regularly, collaborating with nursing staff as needed.
6. Educate clients and their families on rehabilitative care and activities to promote health, safety, and independence.
7. Accurately document observations, interventions, and evaluations using electronic medical records on a tablet.

Qualifications:

1. Current Pennsylvania Physical Therapist - PT license.
2. At least one year of recent experience as a PT.
3. Graduation from an accredited program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
4. Ability to work independently and manage time effectively.
5. Strong interpersonal, organizational, and problem-solving skills.
6. Solid computer skills; prior EMR experience preferred.
