Physical Therapist, PT, Home Health Visits ($5,000 Sign On Bonus)

$5,000 Sign On Bonus for Full-Time

Location: Northeast Philadelphia area.

As a home care Physical Therapist, you will be an integral member of a multidisciplinary healthcare team providing rehabilitative care and skilled nursing to clients, enabling them to remain at home.

Position: Full-time Physical Therapist performing home health visits in Jamison, PA. Servicing adult and geriatric clients across territories in Northeast Philadelphia.

Responsibilities:

1. Make home visits to clients in designated territories.
2. Perform diagnostic tests and measurements (mobility, strength, balance, safety assessments).
3. Develop and implement individualized care plans, including manual therapies, gait training, and balance interventions.
4. Assess and revise care plans, collaborating with nursing staff as needed.
5. Educate clients and families on rehabilitative activities to promote health and independence.
6. Document observations and interventions accurately using electronic tablets.

Qualifications:

• Current PA Physical Therapist (PT) license.
• Graduation from an accredited PT program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Proficiency with computers; experience with EMR preferred.
