San Diego Home Health in Riverside County, CA, is seeking two full-time Physical Therapists (PTs) to serve North and South Riverside County. The role offers hourly pay of $50–$75 with drive time and mileage reimbursement, plus 23 days PTO and a full benefits package.

The PT will provide home-based therapy, perform evaluations, and coordinate with the care team to restore mobility and independence. Ideal candidates hold a CA PT license, have Home Health experience, and are comfortable working in

• Graduate of an accredited Physical Therapy program.
• Current, active California Physical Therapist license in good standing.
• Minimum of one year of professional Physical Therapist experience preferred.
• Home health experience preferred; clinicians with strong rehabilitation backgrounds are encouraged to apply.
• Familiarity with Homecare Homebase (HCHB) and OASIS documentation is a plus.
• Strong clinical assessment, communication, and organizational skills.
• Valid driver's license, reliable transportation, and current automobile insurance.

What You'll Do

As a Home Health Physical Therapist, you'll provide individualized, skilled therapy services to patients in the comfort of their homes, helping them achieve meaningful rehabilitation goals. Key responsibilities include:
