Orionplacement is seeking a licensed Physical Therapist for in-home, field-based care in Redlands, CA, with a $10,000 hiring bonus. Role includes independent patient visits, treatment planning, and collaboration with an interdisciplinary team.

Active CA PT license required; 1+ year home health experience preferred. Travel across Redlands territory and documentation from the field are essential duties of this position.

Qualifications

• BLS/CPR certification preferred.
• Driver's license and reliable insured vehicle preferred.

Responsibilities

• Evaluate patients in their homes and assess mobility, strength, coordination, pain and functional limitations.
• Develop individualized PT treatment plans based on patient needs and goals.
• Provide therapeutic exercises and interventions to improve mobility, strength, balance and independence.
• Educate patients and caregivers on exercises, safety precautions and progress strategies.
• Monitor patient outcomes and adjust treatment plans as appropriate.
• Collaborate with nurses, physicians, therapists, and other care team members.
• Maintain accurate and timely documentation of patient progress and plans of care.
• Travel throughout the Redlands territory to provide in-home care.

Pay:$48.00 - $71.00 per hour

Location:Redlands, CA - Hybrid. This is a field-based home health role where you will provide care in patients' homes throughout the Redlands territory while completing documentation from the field.

Note:Candidates must have an active California Physical Therapist (PT) license. Home health experience, 1+ year of Physical Therapist experience, a Master's or Doctorate degree in Physical Therapy, BLS/CPR certification, a valid driver's license, reliable transportation, HCHB/Homecare Homebase experience, and OASIS experience are preferred.
