Job summary

Orion Placement is seeking a Physical Therapist for a field-based home health role serving Ramona, Poway, Rancho Bernardo, and surrounding communities. The position offers a hybrid in-home care model with independent field work and a supportive interdisciplinary team.

Responsibilities include evaluating patients in their homes, creating and implementing personalized treatment plans, and educating patients and caregivers. Travel within North County San Diego is required.

Qualifications

• Active California Physical Therapist license required.
• Home health PT experience preferred.
• 1+ year of Physical Therapist experience preferred.
• Master's or Doctorate degree in Physical Therapy preferred.
• Current BLS or hands-on CPR certification preferred.
• Valid driver's license and reliable insured vehicle preferred.
• HCHB/Homecare Homebase experience preferred.
• OASIS experience preferred.

• Develop individualized physical therapy treatment plans based on each patient's needs and goals.
• Provide therapeutic exercises and techniques to improve mobility, strength, function and independence.
• Educate patients and caregivers on exercises, safety precautions, and self-care strategies.
• Monitor patient progress and adjust treatment plans as appropriate.
• Collaborate with nurses, physicians, therapists, caregivers, and other members of the interdisciplinary team.
• Travel throughout the North County San Diego territory to provide in-home care.

$55.00 - $65.00 per hour

Location

Ramona, CA - Hybrid. This is a field-based home health position serving patients throughout Ramona, Poway, Rancho Bernardo, Rancho Peñasquitos, and surrounding communities.

Note

Candidates must have an active California Physical Therapist (PT) license. Home health experience, 1+ year of Physical Therapist experience, a Master's or Doctorate degree in Physical Therapy, BLS/CPR certification, a valid driver's license and reliable transportation, HCHB/Homecare Homebase experience, and OASIS experience are preferred.
