San Diego Home Health is seeking a dedicated Physical Therapist to join our Home Health team in Riverside County, CA. We have two full-time positions—one in North Riverside County and one in South Riverside County—offering hourly compensation of $50–$75 based on experience.

You will conduct home-based evaluations, develop treatment plans, and collaborate with a multidisciplinary team to improve mobility and independence for patients at home.

Competenze

• Graduate of an accredited Physical Therapy program.
• Current, active California Physical Therapist license in good standing.
• Minimum of one year of professional Physical Therapist experience preferred.
• Home health experience preferred; clinicians with strong rehabilitation backgrounds are encouraged to apply.
• Familiarity with Homecare Homebase (HCHB) and OASIS documentation is a plus.
• Strong clinical assessment, communication, and organizational skills.
• Valid driver's license, reliable transportation, and current automobile insurance.
• Ability to independently manage patient visits and complete clinical documentation in a timely manner.

What You'll Do
