Qualifications

• Graduation from an accredited Physical Therapy program (CAPTE-approved or equivalent).
• Active Physical Therapist license in the state of practice.
• Minimum of approximately 9 months of clinical experience preferred (new graduates considered).
• Valid CPR certification required or must be maintained.
• Ability to travel to patient homes as part of job responsibilities.
• Valid driver’s license and reliable transportation.

Responsibilities

The Physical Therapist will be responsible for assessing patient needs, developing individualized treatment plans, and delivering rehabilitation services that support recovery, mobility improvement, and long-term functional independence. This role emphasizes autonomy, flexibility, and strong interdisciplinary collaboration within a home health care setting.

Key Responsibilities
