Job summary

Capitol Home Health is seeking a licensed Physical Therapist to evaluate referred patients in a home health setting, design and implement individualized treatment plans, and work closely with physicians and case managers. The PT should demonstrate strong communication and organizational skills and have reliable transportation.

The role requires an accredited PT degree and active Texas licensure, with a minimum of two years in physical therapy, preferably in community/home health.

Qualifications

• Graduate degree in Physical Therapy from an accredited program.
• Current, unencumbered Texas Physical Therapist license.
• Minimum 2 years of physical therapy experience; home health preferred.
• Excellent verbal and written communication; organized.

POSITION SUMMARY

The Physical Therapist (PT) is responsible for evaluating/assessing needs of referred patients and formulating treatment plans in a home care environment. The PT provides therapy services defined in treatment plans while working cooperatively with physicians and case managers. The PT performs a variety of tasks and is encouraged to use a certain degree of creativity and latitude.

QUALIFICATIONS

• Graduate of a Baccalaureate, Master's or Doctorate program in Physical Therapy program accredited by a Physical Therapy accreditation
• Current and unencumbered licensed to practice as a Physical Therapist within the state of employment.
• Two (2) years of appropriate experience as a Physical Therapist. Community/home health experience is preferred.
• Demonstrates good verbal and written communication and organization
• Possess and maintain valid CPR Must have obtained in-person with a practical skills evaluation as part of the final test.
• Access to reliable transportation.
