Job summary

Capitol Home Health is seeking a Physical Therapist to evaluate the needs of referred patients in a home health setting and develop treatment plans. You will provide therapy services and work cooperatively with physicians and case managers to support patient care, with room for professional judgment and creativity.

The role requires PT licensure in Texas, strong communication skills, and reliable transportation to cover areas surrounding Temple and Central Texas.

Qualifications

• Graduate of a Physical Therapy program accredited by a PT board.
• Current and unencumbered PT license in the state of employment.
• Two years of PT experience; home health preferred.
• Demonstrates good verbal and written communication and organization.
• Access to reliable transportation and in-person CPR with skills evaluation.

Responsibilities

• Evaluate/assess needs of referred patients in a home health setting.
• Formulate treatment plans and provide therapy services as defined in plans.
• Collaborate with physicians and case managers to support patient care.

POSITION SUMMARY

The Physical Therapist (PT) is responsible for evaluating/assessing needs of referred patients and formulating treatment plans in a home care environment. The PT provides therapy services defined in treatment plans while working cooperatively with physicians and case managers. The PT performs a variety of tasks and is encouraged to use a certain degree of creativity and latitude.

QUALIFICATIONS

• Graduate of a Baccalaureate, Master's or Doctorate program in Physical Therapy program accredited by a Physical Therapy accreditation
• Current and unencumbered licensed to practice as a Physical Therapist within the state of employment.
• Two (2) years of appropriate experience as a Physical Therapist. Community/home health experience is preferred.
• Demonstrates good verbal and written communication and organization
• Possess and maintain valid CPR Must have obtained in-person with a practical skills evaluation as part of the final test.
• Access to reliable transportation.
