You will perform assessments, develop rehabilitation goals, educate families, document progress, and collaborate with a multidisciplinary team to ensure safe, effective care in a dynamic home-based setting.

Qualifications

• Minimum nine months experience as a physical therapist.
• Preferred community/home health experience.
• Master’s degree in physical therapy approved by CAPTE accredited physical therapist education program.
• Currently licensed to practice in the state of operation and CPR certified.

Responsibilities

• Perform comprehensive physical assessments and analyze medical histories to evaluate patients' conditions accurately.
