Job summary

HealthFlex is seeking a compassionate Home Health Physical Therapist (PT) to provide skilled services in patients' homes across the Bay Area. You will evaluate, develop care plans, and guide patients toward independent mobility with a patient-centered approach.

The role offers part-time or per diem schedules, with duties including collaboration with physicians, progress evaluation, and tailored therapy plans to restore independence at home.

Qualifications

• Current and unrestricted California PT license required.
• Current AHA CPR certification.
• Valid Driver's License.

Responsibilities

• Perform in-home evaluation and develop a care plan.
• Assist patients with mobility and regain independence in the home.
• Create detailed treatment and care plans for each patient.
• Coordinate with patients' physicians to ensure quality care.
• Evaluate and report patients’ progress.

Position Summary: HealthFlex is seeking a compassionate and experienced Home Health Physical Therapist (PT). By joining our 5 star rated clinical team; you will be providing skilled services to help patients regain their independence in the comfort of their own home.

Schedule: Part Time or Per Diem

Locations: We have needs across all of our territories in the following counties, Alameda, Contra Costa, Santa Clara, San Mateo, San Francisco, Marin, Sonoma, Napa and Solano.
