HealthFlex is seeking a compassionate Home Health Physical Therapist (PT) to join our five-star clinical team. Provide skilled therapy in patients' homes across Northern California to help them regain independence and improve mobility.

Responsibilities include comprehensive home assessments, individualized treatment plans, and coordination with physicians. Part-time or per diem options with flexible scheduling to meet patient needs and a supportive leadership team.

Formación

• Current unencumbered California Physical Therapy license.
• Home health experience preferred.
• CPR certification (AHA) preferred.

Responsabilidades

• Perform home health PT evaluations and develop care plans.
• Assist patients with mobility and regain independence.
• Create detailed, patient-specific treatment plans.
• Coordinate with patients' physicians to ensure quality care; report progress.

Position Summary: HealthFlex is seeking a compassionate and experienced Home Health Physical Therapist (PT). By joining our 5 star rated clinical team; you will be providing skilled services to help patients regain their independence in the comfort of their own home.

Schedule: Part Time or Per Diem

Locations: We have needs across all of our territories in the following counties, Alameda, Contra Costa, Santa Clara, San Mateo, San Francisco, Marin, Sonoma, Napa and Solano.

Responsibilities:

• Perform evaluation in the home and develop a care plan.
• Help patients with mobility, movement, and help regaining their independence.
• Create detailed treatment and care plans for each patient.
• Communicate and coordinate with patients’ physicians to ensure a quality level of care.
• Evaluate and report patients’ progress.

Requirements

Qualifications:

• Current and unrestricted California PT license required.
• Current AHA CPR certification
• Valid Driver's License
