Job summary

ABL Health Care in California is seeking a Full Time Physical Therapist (PT) for our Home Health team in San Jose. You will evaluate patients, develop and implement care plans, and monitor progress in the home setting.

This role requires CA licensure, home health experience, a California driver license, and the ability to travel within a 15-25 mile radius. You’ll work with a supportive clinical team and have access to back-office support and company devices.

Qualifications

• Current CA PT license to practice – required.
• Experience in Home Health or Hospice care – 1 year required.
• Current CA Driver's License.
• Able to travel within a 15-25 mile radius regularly.

Responsibilities

• Provide, coordinate and oversee skilled intermittent care to patients in the home.
• Regularly evaluate and re-evaluate patient status.
• Administer treatments, monitor conditions, and report changes to Physician/Case Manager.
• Initiate, develop, implement and revise the patient's plan of care (POC).
• Address problems identified in the POC.
• Educate patient/family/caregiver about condition and care.
• Communicate with other disciplines as warranted.
• Maintain accurate clinical records and submit documentation using company device.
• Comply with company policies to ensure high standards of care.

Position Overview

We are seeking an experienced, self-driven Full Time Physical Therapist (PT) to join our dedicated Home Health team and provide exceptional care throughout the region. As an Physical Therapist (PT), you will evaluate patients' health, create and implement care plans, and track their progress to maintain the highest standards of care. You'll be working closely with our Clinical Director and Clinical Staff, providing support both remotely and independently in the field.

Responsibilities

• Provide, coordinate and oversee skilled intermittent care to patients and include instruction and education of patients and/or caregivers to promote maximal functional status, in the patient's home.
• Regularly evaluates and re-evaluates patient status.
• Administer treatments, monitor patient conditions, and report signs/symptoms signaling a change in patient condition to Physician, Case Manager, and/or as needed Director of Nursing.
• Initiates, develops, implements and revises the patient's plan of care (POC).
• Addresses all problems identified in the POC.
• Provides educational information to the patient/family/caregiver concerning knowledge deficits or needs regarding the patient's condition.
• Communicates with other disciplines involved in the case as warranted.
• Maintain accurate clinical records and submit documentation according to company policy using a company-provided device.
• Comply with all company policies and procedures to ensure high standards of care.

Requirements

• Current PT License to practice in California – Required
• Experience in Home Health or Hospice care – 1 Year Required

• Able to travel within a 15-25 mile radius on a regular basis.
