ABL Health Care is seeking a dedicated Physical Therapist (PT) for full-time home health in Brentwood. You will evaluate patients, create and implement care plans, and monitor progress in the field with support from our Clinical Director and team.

As part of the Home Health unit, you will work closely with clinicians to deliver high-quality, patient-centered care, travel within a 15–25 mile radius, and utilize company devices to document care plans and outcomes.

Qualifications

• Current CA PT license to practice.
• Experience in Home Health or Hospice care – 1 year.
• Current CA Driver's License.
• Able to travel within a 15–25 mile radius regularly.

Responsabilités

• Provide, coordinate and oversee skilled intermittent care to patients and include instruction and education of patients and/or caregivers to promote maximal functional status, in the patient's home.
• Regularly evaluates and re-evaluates patient status.
• Administer treatments, monitor patient conditions, and report signs/symptoms signaling a change in patient condition to Physician, Case Manager, and/or as needed Director of Nursing.
• Initiates, develops, implements and revises the patient's plan of care (POC).
• Addresses all problems identified in the POC.
• Provides educational information to the patient/family/caregiver concerning knowledge deficits or needs regarding the patient's condition.
• Communicates with other disciplines involved in the case as warranted.
• Maintain accurate clinical records and submit documentation according to company policy using a company-provided device.
• Comply with all company policies and procedures to ensure high standards of care.

Position Overview

We are seeking an experienced, self-driven Full Time Physical Therapist (PT) to join our dedicated Home Health team and provide exceptional care throughout the region. As an Physical Therapist (PT), you will evaluate patients' health, create and implement care plans, and track their progress to maintain the highest standards of care. You'll be working closely with our Clinical Director and Clinical Staff, providing support both remotely and independently in the field.
