Job summary

Seaport Scripps Home Health is seeking a licensed Physical Therapist to provide in-home therapy services across the South Coastal Territory. The PT will deliver care in accordance with a physician’s plan, helping patients regain mobility and independence.

Responsibilities include evaluating functional status, developing care plans, and collaborating with physicians while documenting progress and discharge planning. Position is full-time in the San Diego area.

Qualifications

• Licensed California Physical Therapist.
• Minimum 2 years PT experience, home health preferred.
• Strong communication and organizational skills.
• Current CPR certification.
• Valid driver’s license and insured vehicle.

Responsibilities

• Evaluate patients’ functional status and develop care plans
• Assess home environments and recommend modifications
• Provide direct physical therapy treatments
• Educate patients and caregivers on therapeutic techniques and equipment
• Monitor progress and adjust treatment plans
• Collaborate with physicians and interdisciplinary teams
• Document clinical notes and progress reports
• Participate in discharge planning
• Supervise Physical Therapy Assistants per policy and regulations
