Job summary

Seaport Scripps Home Health in San Diego County seeks a licensed Physical Therapist to deliver in-home therapy services across the South Coastal Territory. The PT will implement patient plans, assess environments, and supervise assistants in accordance with physician directives.

Responsibilities include performing evaluations, educating patients and caregivers, documenting progress, coordinating with physicians, and participating in discharge planning.

Qualifications

• Active California Physical Therapy License.
• Minimum 2 years of physical therapy experience (home health preferred).
• Current CPR certification.

Responsibilities

• Evaluate patients’ functional status and develop care plans.
• Assess home environments and recommend modifications.
• Provide direct physical therapy treatments.
• Educate patients and caregivers on therapeutic techniques and equipment.
• Monitor progress and adjust treatment plans.
• Collaborate with physicians and interdisciplinary teams.
• Document clinical notes and progress reports.
• Participate in discharge planning.
• Supervise Physical Therapy Assistants per policy and regulations.
