We are growing and looking for a dedicated Physical Therapist (PT) to join our team. Shifts available: PRN & FT in Northeast, Northwest, and Southwest service areas.

Qualifications

• Texas PT license required or eligible for licensure.
• Home health experience highly preferred.
• Understanding of home health regulations and compliance.

Responsibilities

• Facilitate rehabilitation for patients with disabilities.
• Implement physical therapy care plans.
• Coordinate care with the home health team.
• Educate patients and families about health conditions.
• Prepare timely, complete clinical notes.
• Communicate effectively with patients, families, and care team.

Physical Therapist (PT)

Shifts Available: PRN & FT

Service Areas with Openings: Northeast, Northwest, Southwest

Responsibilities

• Facilitate the rehabilitation of patients with emotional, physical or mental disabilities.
• Implement physical therapy care plans.
• Coordinate care with the home healthcare team.
• Educate patients and family members about health conditions.
• Prepare timely, complete, and accurate clinical notes.
• Communicate effectively with patients, caregivers, healthcare providers, office support, and other care team members.

Qualifications

• Physical Therapist in the state of Texas.
• Home health experience highly preferred.
• Knowledge of home health regulations and general healthcare compliance.
• Experience with HCHB EMR. (preferred but not required)
• Able to handle physical workload.
• Good driving record.
• Excellent communication and people skills to effectively interact with patients, families, and healthcare team members.
