Job summary

Transition Home Healthcare in Houston, TX is seeking a dedicated Physical Therapist Assistant (PTA) to join our home health team. Shifts available: PRN and Full Time; provide rehabilitation services in patients' homes and coordinate with care teams.

You will educate families, document care, and support patients through recovery, with focus on quality, safety, and compassionate care in the community.

Qualifications

• Assistant Physical Therapist in the state of Texas.
• Home health experience preferred.
• Knowledge of home health regulations and general healthcare compliance.
• Experience with HCHB EMR preferred.
• Able to handle physical workload.
• Good driving record.
• Excellent communication and people skills to effectively interact with patients, families, and healthcare team members.

Responsibilities

• Assist in the implementation of physical therapy care plans.
