Job summary

Living Hope Home HealthCare in Houston seeks a Licensed Physical Therapist Assistant to join our home health team. Responsibilities include evaluating patients, developing and implementing therapy plans, and coordinating with an interdisciplinary team to restore or maintain patient well-being.

Qualifications include a Bachelor of Science in Physical Therapy and a current Texas State License as a Physical Therapist Assistant, with a minimum of 1 year in home health, acute care, or rehabilitation

Qualifications

• Therapy skills aligned with standards of practice.
• Strong communication and interpersonal abilities.
• CPR and Hepatitis profile current.
• Knowledge of durable medical equipment.
• Spanish language skills are a plus.

Responsibilities

• Perform initial and ongoing clinical assessments to identify patient needs and determine function.
• Develop and revise a plan of care in collaboration with physicians and care team.
• Provide therapeutic treatments and use adaptive equipment as needed.
• Evaluate outcomes and discharge plans.
• Apply infection control and standard precautions during patient care.
• Supervise PTA as needed.
• Complete timely documentation.
• Meet mandatory continuing education requirements.
• Advise and consult with families and staff.
• Demonstrate commitment, professional growth, and adherence to agency policies.

Job Summary:

• A Licensed Physical Therapist professional who provides coordinated care topatients, evaluates patients and restores or maintains patient well-being according to the plan of care
• Evaluate and provide a treatment plan for a patient with physical, psycho- socialand/or cognitive impairments
• Provide intervention to the deficit and elicit responses
• Collaborates with interdisciplinary team

Job Qualifications:

• Education: Bachelor of Science in Physical Therapy
• Licensure: Current Texas State License as a Physical Therapist Assistant

Experience:

• 1 year of minimum experience as a Physical Therapist in Home Healthcare or Acute care or Rehabilitation setting

Skills:

• Therapy skills as defined as generally accepted standards of practice.
• Good communication and interpersonal skills.
• Proof of current CPR and Hepatitis profile.
• Knowledge of durable medical equipment
• Spanish is a Plus

Essential Functions:

• Perform initial and ongoing clinical assessments to identify patient needs,determine the level of function at appropriate time points and the appropriateness ofpatient for Home health
• Develop and revise a plan of care in consultation with the physician and othercare team members
• Provide therapeutic treatments, evaluate need for and use of adaptive equipmentand environmental modifications, fabricate and maintain adaptive equipment and hand splints as applicable
• Evaluate outcomes of care with appropriate discharge planning.
• Apply concepts of infection control and standard precautions incoordinating/performing patient care activities to prevent contamination andtransmission of disease
• Accept clinical assessments that are consistent with education and competenceto care for patients.
• Supervise PTA as needed
• Complete appropriate documentation in a timely manner
• Meet mandatory continuing education requirements of the agency/license board
• Use effective interpersonal relations and communication skills to advise andconsult with the family and other agency personnel as appropriate
• Demonstrate commitment, professional growth, and competency
• Promote Agency philosophy and administrative policies to ensure quality of care
