Job summary

Transition Home Healthcare in Houston, TX is seeking a dedicated Physical Therapist (PT) to join our home health team. Shifts are available PRN and FT, serving North, Northeast, Northwest, Southwest, and Inner Loop service areas.

You will implement care plans, educate patients and families, and coordinate with the home health team to ensure high-quality patient care. We offer competitive pay, mileage reimbursement, scrubs, PTO/holidays, comprehensive medical benefits, and 401(k).

Qualifications

• Texas-licensed Physical Therapist in good standing.
• Home health experience preferred.
• Knowledge of home health regulations and compliance.
• Experience with HCHB EMR is a plus.
• Able to manage physical workload and driving.
• Excellent communication and patient education skills.

Responsibilities

• Facilitate the rehabilitation of patients with emotional, physical or mental disabilities.
• Implement physical therapy care plans.
• Coordinate care with the home healthcare team.
• Educate patients and family members about health conditions.
• Prepare timely, complete, and accurate clinical notes.
• Communicate effectively with patients, caregivers, healthcare providers, office support, and other care team members.

Responsibilities:

• Excellent communication and people skills to effectively interact with patients, families, and healthcare team members.
