Job summary

Transition Home Healthcare is seeking a dedicated Physical Therapist (PT) for Houston, TX. Shifts available: PRN and FT. Serve diverse service areas including North, Northeast, Northwest, Southwest, and Inner Loop.

The role focuses on rehabilitating patients, implementing care plans, coordinating with the home health team, and educating families. Minimum Texas PT license; home health experience preferred; strong communication essential.

Qualifications

• Must hold Texas PT license.
• Experience in home health is preferred.
• Familiarity with HCHB EMR is a plus.
• Clean driving record is required.
• Strong communication with patients and team is essential.

Responsibilities

• Facilitate rehabilitation of patients with physical disabilities.

Responsibilities

• Facilitate the rehabilitation of patients with emotional, physical or mental disabilities.
• Implement physical therapy care plans.
• Coordinate care with the home healthcare team.
• Educate patients and family members about health conditions.
• Prepare timely, complete, and accurate clinical notes.
• Communicate effectively with patients, caregivers, healthcare providers, office support, and other care team members.

Qualifications

• Physical Therapist in the state of Texas.
• Home health experience highly preferred.
• Knowledge of home health regulations and general healthcare compliance.
• Experience with HCHB EMR. (preferred)
• Able to handle physical workload.
• Good driving record.
• Excellent communication and people skills to effectively interact with patients, families, and healthcare team members.
