Job summary

Transition Home Healthcare is seeking a dedicated Physical Therapist (PT) to join our Houston, TX team. This role supports home health patients across multiple service areas with both PRN and full-time shifts.

Responsibilities include designing and implementing PT care plans, educating patients and families, and coordinating with the home health team. Texas PT licensure, strong communication, and ability to handle physical workload are essential.

Qualifications

• Licensed Texas Physical Therapist (PT).
• Experience with home health preferred.
• Strong communication and patient education skills.

Physical Therapist (PT)

Shifts Available: PRN & FT

Service Areas with Openings: North, Northeast, Northwest, Southwest, Inner Loop

Qualifications:

• Physical Therapist in the state of Texas.
• Home health experience highly preferred.
• Knowledge of home health regulations and general healthcare compliance.
• Experience with HCHB EMR. (preferred)
• Able to handle physical workload.
• Good driving record.
• Excellent communication and people skills to effectively interact with patients, families, and healthcare team members.
