Job summary

Transition Home Healthcare in Houston, TX is seeking a dedicated Physical Therapist (PT) to join our patient-centered home health team. This role supports rehabilitation, develops and implements care plans, coordinates with the home health team, and educates patients and families.

• Excellent communication and people skills to interact with patients, families, and healthcare team members.
