Job summary

Transition Home Healthcare in Houston, TX is seeking a dedicated Physical Therapist (PT) to join our growing home health team. We offer Full-Time, Part-Time, and PRN shifts across Northeast, Northwest, Southeast, and Southwest service areas.

We pride ourselves on compassionate care and a supportive work environment. The PT will implement care plans, coordinate with the home health team, educate patients and families, and document clinical notes to ensure high-quality patient outcomes.

Qualifications

• Texas licensed Physical Therapist (PT) in good standing.
• Home health experience preferred.
• Knowledge of home health regulations and healthcare compliance.
• Experience with HCHB EMR is preferred.
• Able to handle physical workload.
• Good driving record.
• Excellent communication and people skills with patients and care teams.

Responsibilities

• Facilitate the rehabilitation of patients with emotional, physical or mental disabilities.
• Implement physical therapy care plans.
• Coordinate care with the home health team.
• Educate patients and family members about health conditions.
• Prepare timely, complete, and accurate clinical notes.
• Communicate effectively with patients, caregivers, healthcare providers, and the care team.
