HealthFlex Home Health & Hospice is seeking a Home Health Physical Therapist (PT) to deliver high-quality, patient-centered care in home settings across California. You will assess mobility, design tailored plans, and implement therapies to improve function and independence for patients in their homes.

$130,000 to $190,000 per year.
