Job description

BAYADA Home Health Care is currently seeking a Physical Therapist (PT) for a part-time opportunity performing home health visits. This office services adult and geriatric clients in the Gaston and Lincoln county areas.

As a home care Physical Therapist, you will be an integral member of a multi-disciplinary healthcare team that provides rehabilitative care and skilled nursing to clients, enabling them to remain at home.

Responsibilities include:

• Make home visits to clients in designated territories.
• Perform diagnostic tests and measurements, such as mobility, joint range of motion, transfer status, stability, ambulation patterns, muscle strength and endurance, balance, and safety assessments.
• Implement protective interventions to prevent wound formation and promote healing using evidence-based dressings and topical agents.
• Develop and revise individualized care plans, including manual exercises, gait training, and balance interventions.
• Assess and update the care plan regularly and collaborate with nursing in multidisciplinary planning.
• Educate clients and family members on rehabilitative activities to promote health, safety, and independence.
• Document observations, interventions, and evaluations accurately using a touch pad tablet.

Qualifications include:

• Current NC Physical Therapist (PT) license.
• Graduation from an APTA-approved program or equivalent.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Proficiency with computers; experience with EMR preferred.
