Job Opportunity: Physical Therapist (PT) at BAYADA Home Health Care

Position: Part-time Physical Therapist (PT) for home health visits, assessments, and admissions.

Location: Serving adult and geriatric clients in the Gaston and Lincoln county areas.

Role Overview: As a home care Physical Therapist, you will be a vital part of a multidisciplinary team providing rehabilitative care and skilled nursing to enable clients to remain at home.

Responsibilities:

1. Perform home visits within designated territories.
2. Conduct diagnostic tests and measurements, including mobility, joint range, stability, ambulation patterns, muscle strength, balance, and safety assessments.
3. Implement wound prevention and repair interventions using evidence-based dressings and topical agents.
4. Create and update individualized care plans, including manual exercises, gait training, and balance interventions.
5. Assess and revise care plans regularly, collaborating with nursing staff as needed.
6. Educate clients and families on rehabilitative activities to promote health, safety, and independence.
7. Document observations, interventions, and evaluations accurately using electronic devices.

Qualifications:

1. Current NC Physical Therapist (PT) license.
2. Graduation from an APTA-approved program or accredited by the Committee on Allied Health Education and Accreditation.
3. Ability to work independently and manage time effectively.
4. Strong interpersonal, organizational, and problem-solving skills.
5. Proficiency with computer skills; experience with electronic medical records preferred.
