Zusammenfassung

Best Home Health Providers Inc. is seeking a licensed Physical Therapist to join our California home health team.

This per diem role focuses on delivering skilled PT services in patients' homes, helping them regain mobility and independence while coordinating with physicians and caregivers. Requirements include a PT degree (Bachelor’s or Master’s), active CA license, CPR certification, and a minimum of two years of PT experience, ideally in home health.

Qualifikationen

• Must have PT degree (Bachelor/Master) from an accredited program.
• Active CA Physical Therapy license is required.
• Minimum 2 years PT experience; home health background is a plus.
• Current CPR certification (AHA).
• Valid driver’s license with reliable vehicle is required.

Aufgaben

• Evaluate patients’ functional status, mobility, strength, gait, and ROM to create individualized plans.
• Deliver PT services per physician plan of care and CA PT Practice Act.
• Educate patients and families on exercises, equipment use, and goals.
• Collaborate with physicians and care team; report changes and participate in case conferences.
• Supervise PTAs and oversee Home Health Aide care when PT is sole skilled service.
• Manage clinical documentation and discharge planning.
• Identify needs for additional home health services and coordinate referrals.

Please note that this role is per diem with a rate of $160 - $190 per visit.

Responsibilities

• Evaluate patients’ functional status, mobility, muscle strength, gait, and ROM to develop individualized treatment plans.
• Deliver physical therapy services in accordance with the physician’s plan of care and California’s Physical Therapy Practice Act.
• Educate patients and their families or caregivers on therapeutic exercises, equipment use, and recovery goals.
• Collaborate with physicians and the care team—reporting changes in patient condition and participating in case conferences.
• Supervise Physical Therapy Assistants and, when PT is the sole skilled service, oversee Home Health Aide care bi-weekly.
• Manage documentation including clinical progress summaries and discharge planning.
• Identify needs for additional home health services and coordinate referrals as appropriate.
• Perform other duties as required for patient services.

• Bachelor’s or Master’s degree in Physical Therapy from an accredited program.
• Active California Physical Therapy license.
• Minimum 2 years of PT experience; home health background is a plus.
• Strong communication skills and the ability to connect with patients and families.
• Current CPR certification with AHA.
• Valid driver’s license and reliable, insured vehicle.
