Job summary

Hemet Hills Post Acute in Hemet, CA, is seeking a part-time Physical Therapist. You will evaluate patients, develop individualized treatment plans, and guide therapeutic exercises to improve function and mobility.

Responsibilities include documenting progress, using hands-on techniques, and advising families on home exercise programs and recovery expectations. CA PT license and post-acute experience preferred.

Qualifications

• Must be a graduate of an accredited Physical Therapy Program.
• Experience in post-acute or long-term care environment preferred.
• Current, unencumbered CA Physical Therapy License required.
• CPR certification may be requested.
• COVID vaccine required or valid exemption.

Responsibilities

• Consult with patients to assess condition and review medical history.
• Diagnose movement dysfunction and plan individualized treatments.
• Coach patients with therapeutic exercises and stretches.
• Provide hands-on therapy such as massage to reduce pain.
• Maintain patient records and adjust plans as progress occurs.
• Advise patients and families on at-home treatment and recovery expectations.
