We are seeking an experienced Physical Therapist to join our team. The successful candidate will be responsible for providing high-quality patient care that helps improve patients' lives in the comfort of their homes.

The Physical Therapist evaluates and treats residents under physician orders, using various therapeutic procedures to restore muscle strength, coordination, body alignment, relieve pain, and improve movement and function. The Physical Therapist collaborates as part of the interdisciplinary team, providing support and information related to Physical Therapy.

Responsibilities

• Performs physical therapy assessments, diagnostic tests, skilled procedures, and ongoing evaluations for clients receiving services under a medically approved Plan of Care.
• Completes initial assessments within 48 hours of referral, unless otherwise arranged.
• Develops and updates therapy care plans, reviews, and documents all findings, plans, interventions, and outcomes, submitting documentation per agency policy.
• Communicates plans and changes to physicians, client Case Managers, and caregivers through care plans, notes, and meetings.
• Implements client education tailored to individual needs and environment, assessing knowledge and planning, teaching, and evaluating effectiveness.
• Participates in interdisciplinary team meetings, discharge planning, and shares expertise to improve patient outcomes.
• Contributes to ongoing quality improvement activities, including use of assessment tools and outcome reports.
• Ensures safety and infection control practices are followed, recognizing and responding to unsafe situations.
• Supervises home care team members, including home health aides and physical therapy assistants, evaluating performance and providing feedback.
• Performs other duties as necessary, ensuring compliance with agency policies, standards, and ongoing professional development.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current license/registration as a Physical Therapist in practice state(s).
• Minimum two years of healthcare experience; home care experience preferred.
• Current CPR certification.
• Strong communication and interpersonal skills.
• Ability to work independently with minimal supervision.
• Licensed driver with insured vehicle and good driving record.
