We are seeking an experienced Physical Therapist to join our team. The successful candidate will be responsible for providing high-quality patient care that will help change patients' lives for the better at the comfort of their homes.
Responsibilities
- Performs physical therapy assessments, diagnostic tests, skilled procedures, and ongoing evaluation for clients under a medically approved Plan of Care. Completes initial assessments within forty-eight (48) hours of referral, unless otherwise arranged. Documents all findings, plans, interventions, and outcomes, and submits documentation to the Agency within seven (7) days or per Agency policy. Communicates plans and changes to the physician, Case Manager, and other caregivers.
- Implements teaching appropriate to the client’s needs and environment. Assesses knowledge level and learning needs, plans, teaches, supervises, and counsels clients to promote improvement, symptom management, and resource utilization. Evaluates teaching effectiveness and updates care plans accordingly. Participates in interdisciplinary team meetings and discharge planning.
- Assists in developing, implementing, and evaluating quality improvement processes. Uses assessment tools and outcome reports to enhance therapy services and client outcomes. Ensures consistency in data collection with the interdisciplinary team.
- Promotes safety and infection control, recognizing unsafe situations, and using equipment safely. Assesses environmental safety and takes preventive measures.
- Supervises home care team members, including home health aides and physical therapy assistants, evaluating performance and providing feedback.
- Performs other duties as necessary, complying with policies, standards, and attending required educational programs. Demonstrates commitment to personal and professional development.
Qualifications
- Graduate of an accredited Physical Therapy program.
- Current license/registration as a Physical Therapist in the state(s) of practice.
- Minimum two (2) years of healthcare experience; home care experience preferred.
- Current CPR certification.
- Strong communication and interpersonal skills.
- Ability to work independently.
- Licensed driver with insured vehicle and good driving record.
