We are seeking an experienced Physical Therapist to join our team.
The successful candidate will be responsible for providing high-quality patient care that helps improve patients' lives in the comfort of their homes.
Responsibilities
- Perform physical therapy assessments, diagnostic tests, skilled procedures, and ongoing evaluations for clients under a medically approved Plan of Care. Complete initial assessments within 48 hours of referral, or as otherwise arranged. Develop and review therapy care plans, document all findings, and communicate effectively with physicians, case managers, and caregivers. Update schedules and physician orders promptly.
- Implement patient education tailored to individual needs, assess and address learning needs, and promote recovery and symptom management. Contribute to interdisciplinary team meetings, share expertise, and participate in discharge planning.
- Assist in quality improvement initiatives, utilize assessment tools like OASIS, and participate in team efforts to enhance therapy services and outcomes. Ensure consistency in data collection and documentation.
- Promote safety and infection control, recognize unsafe situations, and ensure a safe environment for clients and staff. Use equipment safely and evaluate environmental safety.
- Supervise home care team members, including home health aides and therapy assistants, providing evaluations and timely reviews as delegated.
- Perform other duties as necessary, ensuring compliance with agency policies and professional standards, and participate in educational activities for ongoing professional development.
Qualifications
- Graduate of an accredited Physical Therapy program recognized by the American Physical Therapy Association or equivalent.
- Current license/registration as a Physical Therapist in applicable states.
- Minimum of two years of healthcare experience; home care experience preferred.
- Current CPR certification.
- Strong communication and interpersonal skills.
- Ability to work independently with minimal supervision.
- Valid driver’s license with insured vehicle and a good driving record.
