Job description

We are seeking an experienced Physical Therapist to join our team.

The successful candidate will be responsible for providing high-quality patient care that helps improve patients' lives in the comfort of their homes.

Responsibilities
  • Perform physical therapy assessments, diagnostic tests, skilled procedures, and ongoing evaluations for clients receiving services under a medically approved Plan of Care. Complete initial assessments within 48 hours of referral unless other arrangements are made.
  • Develop therapy care plans, review and update them as needed, and document all findings, plans, interventions, and outcomes within the agency's specified timeframe.
  • Communicate plans and changes to physicians, case managers, and caregivers through care plans, progress notes, and participation in care conferences.
  • Implement teaching tailored to the client's needs, assess knowledge levels, and educate clients about their therapy and available community resources.
  • Participate in interdisciplinary team meetings, share expertise, and contribute to discharge planning.
  • Assist in quality improvement processes, utilize assessment tools like OASIS, and participate in data collection to improve client outcomes.
  • Promote safety and infection control practices, recognize unsafe situations, and ensure a safe environment for clients and staff.
  • Supervise home care team members, including aides and assistants, and evaluate their performance.
  • Perform other duties as necessary, ensuring compliance with agency policies, standards, and regulations.
Qualifications
  • Graduate of an accredited Physical Therapy program by the APTA or equivalent.
  • Current license/registration as a Physical Therapist in the practicing state(s).
  • At least two years of healthcare experience, with home care experience preferred.
  • Current CPR certification.
  • Strong communication, interpersonal, and independent working skills.
  • Licensed driver with insured vehicle and a good driving record.
